How is the wheel designed for running?

It is a hamster wheel toy for small animals, rather than a general cage accessory.

Can I use it in or outside a cage?

Yes, the wheel can provide exercise inside or outside the cage, and it can be securely hung on mesh cage sides. That hanging option is specifically for mesh sides, so consider the cage construction when deciding how to position it.
